Efektivitas madu sebagai balutan (wound dressing) ulkus diabetikum telah banyak diteliti, di mana hasi studi in vitro dan studi pada hewan menunjukkan efek antibakterial, antiinflamasi, antioksidan, dan debridemen autolitik dari madu. Sementar itu, studi in vivo pada manusia masih terus dikembangkan.
Salah satu komplikasi umum diabetes mellitus adalah ulserasi dan infeksi kaki. Ulkus diabetikum (diabetic foot ulcer) diketahui merupakan penyebab utama dari amputasi kaki dan kecacatan pada pasien diabetes. Selain itu, ulkus ini juga dilaporkan memiliki mortality rate 5 tahun yang bisa mencapai 42%.
Tata laksana yang tepat dibutuhkan untuk mengurangi risiko kecacatan dan kematian pada pasien ulkus diabetikum. Pada umumnya, standar tata laksana ulkus ini meliputi debridemen, wound dressing yang lembab, penurunan beban luka (wound off-loading), pemeriksaan vaskular, terapi infeksi, dan kontrol glikemik.
